Sacred Origins Health (ABN 73 687 428 728) is a naturopathy and massage practice on the Central Coast of NSW, run by Kirsten. This policy explains how we collect, use, store and share your personal information, including health information, when you visit our website, book with us, become a client, or use our client app. We handle your information in line with the Privacy Act 1988 (Cth), the Australian Privacy Principles, and the Health Records and Information Privacy Act 2002 (NSW).
We collect this information directly from you, with your consent. You can choose not to give us some information, but we may then be unable to provide safe, appropriate care.
We do not sell your information, and we do not use it for advertising.
Your health records are seen only by Kirsten, as your practitioner. We share information with others only when you ask us to (for example, a letter to your GP), when it is needed for your care and you agree, or when the law requires or allows it (for example, a serious threat to someone's life, health or safety).
We use trusted service providers to run our systems. They store or process information on our behalf and are not permitted to use it for their own purposes:
Because of this, some of your information may be stored outside Australia. We choose providers that protect information with encryption and access controls, and we take reasonable steps to ensure they handle it consistently with Australian privacy law.
Our app can only be reached over an encrypted (HTTPS) connection, sign-in is through your Google account, and only Kirsten's practitioner account can view client records. Access to our systems is limited to the people who run the practice. We keep health records for at least seven years after your last consultation (or, for a client under 18, until they turn 25), as NSW law requires, and then securely delete them.
You can ask to see or get a copy of the information we hold about you, and ask us to correct anything that is wrong. Email us and we will respond within 30 days. In rare cases the law allows us to refuse access, and if so we will tell you why in writing.
